Serpent Belt - Simka the Snake

You have resistance to poison damage while
wearing this belt.
As a bonus action, you can command this belt to
come alive, taking the form of Simka the Snake
that follows your verbal commands for an hour.
If Simka the Snake is killed, or moves 60ft away
from the wearer, it reverts to belt form.
If reverted to belt form from one of the two
actions above, the belt cannot transform again
until the user has completed a long rest.
