When Fear enters, target player sacrifices a creature. If that target was an opponent, you lose 2 life.

Whenever a creature dies, put a madness counter on Fear.

Sacrifice Fear: Mill X cards, where X is the number of madness counters on Fear. Then return a creature card with mana value X or less from your graveyard to the battlefield tapped.
